Whether your visit is for a routine vision exam, contact lenses and eyewear, or for medical or surgical services, we look forward to having the opportunity to serve you. To ensure your first visit runs smoothly, we would like to provide you with information on our office practices and procedures.
We will call you 1-2 days before your visit to remind you of your scheduled time. If you need to cancel or change your appointment please let us know as soon as possible so we can reschedule your appointment.
Please bring your insurance card(s) with you so we can make a copy for our files.
Please check with your insurance carrier to determine if you need a referral to see us. If you do, it is your responsibility to obtain your referral prior to being seen. Plan to arrive 10-15 minutes before your scheduled visit to complete the preliminary paperwork and not delay your time with the Doctor.
If your insurance plan requires a co-payment, this will be collected at the time of your visit. It is not our practice to bill patients for co-payments.
We participate with most but not all major insurers. Please check with us and we will let you know if we participate with your company. Regardless of participation, we welcome you to our practice.
